Introduction
Evidence synthesis (ES) is often required for economic evaluation (EE) of pharmaceuticals. Commonly used methods are based on the assumption of proportional hazards in trial data, using the hazard ratio (HR). Alternative methods for ES are increasingly used in EE, in situations where the pattern of hazards in the trial data indicates that the proportional hazards assumption may be violated. The impact of these methodological choices on model outcomes is explored.
MethodsA network of trials of BRAF-targeted treatments for advanced melanoma, derived using a systematic review of the literature, is chosen for the study. Guyot's method is used to create individual-patient Kaplan-Meier (K-M) data from published survival curves. Log-cumulative hazard plots and Schoenfeld residuals are derived to examine patterns in hazards within the trial data. All analyses are conducted in R version 3.5.0©. Three alternative methods for ES are tested: 1) Network meta-analysis (NMA) based on published HRs and the assumption of proportional hazards. 2) NMA using fractional polynomials (FP) based on digitised K-M data, allowing the relaxation of the proportional hazards assumption. 3) NMA using an accelerated failure time (AFT) model based on digitised K-M data, allowing the relaxation of the proportional hazards assumption. The derived estimates of relative efficacy from each method are applied in a partitioned survival cost-effectiveness model programmed in Microsoft Excel™.
ResultsThe model outcomes predicted by each method (HR, FP and AFT) are presented and compared. Both deterministic and probabilistic results are presented, alongside a discussion around how the uncertainty in these structural assumptions may be captured in EE.
ConclusionsStructural assumptions in ES may lead to differences in model outcomes. The impact of these differences may be important in situations where decision uncertainty is high. Methods should be chosen and justified based on patterns of hazard present in the trial data.
Design and Manufacturing of Tendon-Driven Soft Foam Robots

A design and manufacturing method is described for creating a motor tendon–actuated soft foam robot. The method uses a castable, light, and easily compressible open-cell polyurethane foam, producing a structure capable of large (~70% strain) deformations while requiring low torques to operate (<0.2 N·m). The soft robot can change shape, by compressing and folding, allowing for complex locomotion with only two actuators. Achievable motions include forward locomotion at 13 mm/s (4.3% of body length per second), turning at 9◦/s, and end-over-end flipping. Hard components, such as motors, are loosely sutured into cavities after molding. This reduces unwanted stiffening of the soft body. This work is the first demonstration of a soft open-cell foam robot locomoting with motor tendon actuators. The manufacturing method is rapid (~30 min per mold), inexpensive (under $3 per robot for the structural foam), and flexible, and will allow a variety of soft foam robotic devices to be produced.

Verifying Timestamps of Occultation Observation Systems

We describe an image timestamp verification system to determine the exposure timing characteristics and continuity of images made by an imaging camera and recorder, with reference to Coordinated Universal Time. The original use was to verify the timestamps of stellar occultation recording systems, but the system is applicable to lunar flashes, planetary transits, sprite recording, or any area where reliable timestamps are required. The system offers good temporal resolution (down to 2 ms, referred to Coordinated Universal Time) and provides exposure duration and interframe dead time information. The system uses inexpensive, off-the-shelf components, requires minimal assembly, and requires no high-voltage components or connections. We also describe an application to load fits (and other format) image files, which can decode the verification image timestamp. Source code, wiring diagrams, and built applications are provided to aid the construction and use of the device.

Ultraviolet Light Disinfection of Hospital Water for Preventing Nosocomial Legionella Infection: A 13-Year Follow-Up

Background and Objective:
CDC has estimated that 23% of Legionella infections are nosocomial. When a new hospital was being constructed and a substantial increase in transplantation was anticipated, an ultraviolet light apparatus was installed in the water main of the new building because 27% of water samples from taps in the old hospital contained Legionella. This study reports the rate of nosocomial Legionella infection and water contamination since opening the new hospital.
Methods:Charts of all patients with positive Legionella cultures, direct immunofluorescent antibody (DFA), or urine antigen between April 1989 and November 2001 were reviewed. Frequencies of DFAs and urine antigens were obtained from the laboratory.
Results:None of the 930 cultures of hospital water have been positive since moving into the new building. Fifty-three (0.02%) of 219,521 patients had a positive Legionella test; 41 had pneumonia (40 community acquired). One definite L. pneumophila pneumonia confirmed by culture and DFA in August 1994 was nosocomial (0.0005%) by dates. This patient was transferred after prolonged hospitalization in another country, was transplanted 11 days after admission, and developed symptoms 5 days after liver transplant. However, tap water from the patient's room did not grow Legionella. Seventeen (2.5%) of 670 urine antigens were positive for Legionella (none nosocomial). Thirty-three (1.2%) of 2,671 DFAs were positive, including 7 patients (21%) without evidence of pneumonia and 6 (18%) who had an alternative diagnosis.
Conclusion:Ultraviolet light usage was associated with negative water cultures and lack of clearly documented nosocomial Legionella infection for 13 years at this hospital.

Summary
Introduction
Important questions concerning the structure and operation of a European central bank (ECB) remain to be answered. How much independence should national central banks retain during the transition to a single currency? What voting or mediation rules should be used to resolve conflicts among the national representatives on the ECB's governing council? What role should be played by existing central banks in implementing pan-European policies once the ECB comes into operation?
The Delors Report and the provisional statutes of the ECB, drafted by the governors of European Community central banks in Basel in November 1990, provide clearer answers to some of these questions than others. According to the Delors Report, during the transition to a single central bank (‘Stage 2’ of the process of monetary unification in the language of Brussels), national central banks will retain full nominal independence in the sense of continuing to issue their own national currencies and to intervene in domestic financial markets, but little real autonomy in that exchange rates will become immutably fixed and hence money supplies and interest rates will be determined by market forces. According to the draft statutes of the ECB, the policies of the new institution will be decided by votes cast by members of the bank's council, consisting of the 12 governors of the existing central banks and 6 executive directors appointed by the European Council. Voting will be by simple majority.

The laser Raman microprobe was used to interrogate individual fly ash particles as small as 1 μm diameter and record “fingerprint” Raman spectra from both crystalline and non-crystalline components.of the fly ash. When compared to reference patterns of known crystalline phases, the Raman spectra can be used to identify crystalline phases and can give some structural information on other phases. Furthermore, because this method characterizes fly ash particles on an individual basis, correlations to both color and morphology of the particles can be made.

High strength mortars have been prepared utilizing optimized particle packing, reactive substituents to modify the chemistry- and addition of superplasticizers. Otherwise the processing techniques were conventional. The compressive strengths of one prototype material after curing at temperatures from 38 C to 250 C were above 70 MPa. The strengths were particularly high at 175 C (195 MPa) where excellent bonding had developed; one chemically modified material reached 245 MPa. The specimens cured at 175 and 250 C (after a lower temperature precure) developed their strengths rapidly, having reached essentially full strength by 7 days. At lower curing temperatures the strength increased with time, apparently still increasing at 56 days (106 MPa) for the materials cured at 38 C. Modified mixtures were prepared using different proportions of silica fume, MgO, different ratios of sand to fine components, and different sand mineralogy and other admixture proportions for rheological optimization. Microhardness, dynamic Young's modulus, density, and permeability were measured in addition to strength. Matrix chemistry and sand mineralogy and proportions affected the strength. Matrix-aggregate bond was very important. The above types of cementitious materials have potential importance for applications where they may be exposed to extreme conditions and to temperature cycling.

In the absence of any identified solid phase host (other than the original glass), boron has been assumed to accumulate in the fluid during the reaction of borosilicate glass waste forms with aqueous fluids. Using this assumption, it is possible to define a boron index which can be used to monitor the amount of glass that has been dissolved and to provide a worst-case measure of the degradation of the primary glass waste form. Several boron-containing silicate phases have been identified thus invalidating the assumption that boron does not precipitate. The effect is apparently small and the assumption that boron release is a direct measure of degree of alteration of borosilicate glass is still probably a good one.

Argillaceous rocks, used either as a repository rock or as a backfill material provide a reactive medium with which escaped radionuclides can react. Solutions containing U, Nd Gd, Cs, and Sr were reacted with bulk specimens of two argillites with the objective of determining penetration of heavy metals into the rock, and the reaction products. The two Devonian argillites were chosen to represent a highly reducing rock containing pyrite and carbonaceous material as accessory minerals, and a clean, alumino-silicate rock containing mainly quartz and layer silicates. Cylinders of rock were reacted with solutions at saturated steam pressures and 200°C.
At the end of the runs, both rocks contained reduced uranium in the form of U02.25. The rare earths were associated with the uranium. There was a precipitated coating of uranium-rich material on the outside of the reducing shale and few of the radionuclides had penetrated the interior. Uranium also formed a precipitate within 0.5 mm of the surface of the oxidized shale leaving a depleted zone near the surface. Cesium and strontium did not penetrate the reducing shale but were uniformly distributed throughout the oxidized shale. Chlorite, apatite and pyrite were found to be the most reactive phases in the shales.

The stability of a tailored ceramic waste form under hydrothermal conditions was the subject of several previous papers [1,7–10] from this laboratory. One of the results reported in these studies was the apparent dissolution and reprecipitation of the alkaline-earth molybdate phases (scheelite-structure phase). The composition of the scheelite-structure phase after hydrothermal treatment was different from that before treatment (approximately Ca95 Sr5 after and Ca35 Sr40 Ba25 before). A schematic phase diagram was presented at that time to explain the results obtained.
This paper is a report of our attempt to experimentally determine the phase relations in the alkaline-earth molybdate (Ca, Sr, and Ba) and verify our previous interpretation. For this work, we prepared compositions throughout the ternary system and heated them at 1200°C for periods of 2 to 24 hours. The experimental products were characterized by x-ray diffraction and SEM/EDX examination.
At 1200°C, the two-phase solvus is nearly symmetric and extends from near the pure calcium and barium end-members to about 35 mole percent strontium at the critical point of the solvus. The increase in grain size, uniformity of composition within and between grains of the same phase and the approach to “textural equilibrium” in the 1200°C experiments all strongly suggest that equilibrium was attained.
New, calcium-rich compositions are suggested for the scheelite-structure phases in the supercalcine-ceramics based upon the interpretation of data from this study. This observation forces a re-examination of the assumption concerning the partitioning of Sr and Ba amongst the phases in the supercalcine-ceramic.
